By the beginning of the second volume of the Doctor Sex trilogy, Kurt von Kramer has had his Swiss clinic closed down, and has reappeared as Luís Romero on the Caribbean island of San Trinidad. An abandoned colonial fort has been transformed into a brothel, the location of rampant and often sadistic orgies.

Eventually Juana and Diego have had enough, and escape together into the gardens of the fort, but the police have arrived and Kramer/Romero has vanished …
